REHOBOTH (1420 WBSM) — An Attleboro man has been identified as the person killed in a crash Tuesday afternoon in Rehoboth that involved a van transporting Department of Mental Health patients crashing into a parked truck.

According to the Bristol County District Attorney’s Office, the victim was Steven Lacombe, 54.

The crash happened shortly before 3 p.m. on Route 118. Rehoboth Police said they received a call from an employee at the Attleboro Rehoboth Building Supply store reporting that a van had rear-ended a flatbed truck across the street.

First responders arrived to find five alive but injured victims, including the driver, who were taken to an area hospital. They are all expected to survive, the D.A.’s Office said.

Lacombe, who was seated in the rear passenger’s side seat, was declared deceased at the scene.

According to the D.A.’s Office, the occupants of the van, a Toyota Sienna, were all patients of the Massachusetts Department of Mental Health and were being transported from their daycare facility in Taunton to their group home in Attleboro. The driver was identified by the D.A.’s Office as a 67-year-old Providence, Rhode Island man who has been driving for the DMH for the past six years.

The D.A.’s Office said the preliminary investigation suggests the van was driving north on Route 118 near 44 Park Street and left the roadway for some unknown reason, striking the back of the flatbed truck, which was parked off the road.

“There was no initial evidence the driver applied his brakes prior to impact,” according to a D.A.’s Office release, which noted that the crash remains under investigation and no criminal charges have been filed at this time.
